Gynecomastia surgery is a procedure to remove excess fat, glandular tissue and/or skin from overdeveloped or enlarged male breasts. In severe cases of gynecomastia, the weight of excess breast tissue may cause the breasts to sag and stretch the areola (the dark skin surrounding the nipple). In these cases, the position and size of the areola can be surgically improved and excess skin may need to be reduced. Gynecomastia may result from hormonal changes, heredity, disease, or the use of certain drugs, and can present unilaterally (one breast) or bilaterally (both breasts).

There are a variety of different techniques used by Dr. Asif Bhatti to treat gynecomastia. Gynecomastia surgery can be combined with other forms of body contouring surgery, including liposuction, or performed at the same time with other elective surgeries.

CORRECTION OF GYNECOMASTIA PROCEDURE:

There are four grades of gynecomastia depending on the size of the breast tissue and the amount of skin (Simon Classification).

As part of the pre-operative work up, a full endocrine (hormonal) screen will be employed to ensure there are no underlying causes. A baseline ultrasound scan may also be performed to ensure it is true gyaecomastia (glandular tissue as opposed to pseudo-gynaecomastia which is adipose or fat tissue).

An ultrasound scan will clearly delineate and establish the size of the glandular tissue as well as exclude the presence of any suspicious mass within the breast tissue. Surgery in true gynecomastia entails liposuction and then removal of the glandular tissue immediately after the liposuction. This is usually reformed through an incision placed from 3 to 9 o’clock at the nipple areolar -skin interface. This produces a faint final scar, which is not usually noticeable. The fat under the nipple is also mobilised to prevent any depressions being visible beneath the nipple.

In some circumstances, it may be necessary for skin to be removed. It is necessary to wear a support garment for a period of six weeks post-operatively.

LIPOSUCTION AND OPEN EXCISION

If glandular tissue is present then this will require an open excision to ensure it’s complete removal.

LIPOSUCTION ALONE (PSEUDO-GYNECOMASTIA)

In cases where there is pseudo gynecomastia, then liposuction alone may be adequate to remove the excess adipose tissue.

WHO IS A GOOD CANDIDATE & WHAT IS INVOLVED?

Patients who have excess fat, enlarged breast tissue or ‘puffy’ breasts may benefit from a gynecomastia surgery. In this procedure, excess skin, fat or breast tissue is removed surgically.

WHAT ANAESTHETIC IS USED AND WHAT HOSPITAL STAY IS REQUIRED?

This procedure is most commonly performed under a local anaesthetic using a tumescent fluid, but can be carried out under general anaesthesia at your request. This procedure is performed in a day surgery setting, using a rapid recovery technique. You will be able to return to the comfort of your own home soon after completion of the surgery.

WILL THIS PROCEDURE LEAVES SCARS?

Dr. Asif Bhatti undergoes regular training on the most current surgical methods and will choose the incision option that will minimise scarring. These surgical incisions are made very close to the border of the nipple and should be become barely noticeable in a short time.

RECOVERY:

The recovery period for this procedure will vary from patient to patient and you could return to work within 3-5 days in most situations. Light exercise may be resumed within 4-5 weeks but strenuous exercise should be avoided for at least 6-8 weeks. You will be given post-surgery instructions to follow and if you have any questions, please speak to Dr. Asif Bhatti or his clinic staff.
