Lower eyelid blepharoplasty includes a collection of surgical techniques that aims to improve the appearance of the lower eyelids. Historically, lower blepharoplasty was a reductive procedure in which skin and/or fat was removed in order to reduce lower eyelid wrinkles, skin redundancy, and fat bulges., present trends follow a tissue-preserving philosophy that may include orbital and sub-orbicularis fat repositioning and fat transfer techniques to restore apparent volume loss associated with facial aging. WHO IS A GOOD CANDIDATE & WHAT IS CORRECTED?

Patients who have sagging, wrinkly or excess fat on their lower eyelids may benefit from an lower blepharoplasty. In this procedure, excess skin or fat is removed surgically. There are non-surgical methods available if you only have a mild amount of sagging, wrinkling or hollowness.

WHAT ANAESTHETIC IS USED AND WHAT HOSPITAL STAY IS REQUIRED?

Patient with following indications are normally corrected by lower eyelid blepharoplasty:

  • Rhytidosis and lower eyelid dermatochalasis(excess skin lower eyelid)
  • Relative steatoblepharon( appearance of weight in the lower eyelid
  • Pronounced nasojugal groove( prominent junction of cheek and eyelid lower)
  • Infraorbital/malar deflation(sunken dark eyes)
  • Malar mounds or festoons (elevated  skin of lower eyelid)
  • Lower eyelid asymmetry

WHAT ANAESTHETIC IS USED AND WHAT HOSPITAL STAY IS REQUIRED?

Normally the operation is done under full anaesthesia,but some cases of lower eyelid blepharoplasty can be done under local anesthesia if patient so agrees. Markings are often performed before operation  with the patient in a seated position.

It is done by making incision just below the lower eyelashes or sometimes from within the eye. Any excess skin is removed as well as extra fat is taken care off. In modern lower eyelid blephroplasty there is fat transfer process in which fat is removed from the area of protused orbital layer and add where there is depression.

 WILL THIS PROCEDURE LEAVE SCARS?

As the incision line in the lower eyelid blepharoplasty is just below the eyelashes so it is fairly hidden under it. When fully healed the scar is not noticeable to anyone.

WHAT IS THE RECOVERY PERIOD?

Patient is normally able to go home after fully recovery from anesthesia in 3-4 hours. Cold compresses are often recommended to reduce swelling in the first 48 hours, followed by warm compresses. Bland ointment or ophthalmic antibiotic (or steroid/antibiotic combination) ointment or drops are applied for the first postoperative week. Strenuous activity is avoided for a period of 3 to 5 days  after surgery. Patient are normally advised to wear dark glasses till discharge.

  • The lower eyelid blepharoplasty is fairly safe procedure but patient should be made aware of some complications like Retro bulbar hemorrhage that is a rare but serious complication that should be emergently addressed,undercorrection or overcorrection ,Lagophthalmos and ectropion.
